Circle P's "Peace of Mind" Waiting List Reservation Form

This form is to reserve a spot on the "Peace of Mind" waiting list to claim a Circle P baby in the future. I understand that in addition to this form, a fee of $100 is due within 10 days of submitting this form (via Paypal F&F) to claim one of 10 available spots. By signing this document, I am stating that I have read the information document in its entirety and agree to its terms without questions.

** If you are looking for a baby from specific parents, Circle P cannot guarantee the timeline for a litter from those parents as we are dealing with Mother Nature. However, you have the choice to skip babies until a litter is produced by the desired parents for up to 36 months before a pup must be selected or you must be removed from the list. If for any reason the desired parents are no longer bred for any reason, you will be notified so you can make the choice to select a different baby or remove yourself from the waiting list.
