By signing below, you acknowledge and agree to attend all scheduled sessions with this service provider. If you are unable to attend, we kindly ask that you cancel within the required timeframe. Please note that missed appointments without proper notice may result in a charge from My Sister’s Keeper-Cleveland ranging from $18 to $200, based on the scheduled service. Repeated no-shows or unpaid balances may also affect your ability to receive future services. We appreciate your understanding as we strive to support as many sisters as possible.
