    The information contained in this application is used only for the purpose of placing a puppy and will not be used elsewhere. A $250 holding fee to hold your puppy will be collected after your application is approved, and will be deducted off the total price of the puppy at pickup. Holding fee is nonrefundable. By sending this electronically and placing your signature at the end of this application, I will assume, that you have completely read this questionnaire and comprehend it fully. Additionally, by signing, you understand that applying does not ensure approval. I reserve the right to refuse the sale of one of my puppies to anyone, for any reason, without explanation. Your signature certifies the information is correct, and that you understand that the information can be verified. Please know that these puppies range from $1800-$2500.

  • If you rent, are you approved for a pet?*

  • What gender are you looking for?*
  • Is there a certain coat type/color you are looking for?*
  • Are you purchasing this puppy as a surprise or gift for someone other than your immediate family?*
  • Do you agree to provide occasional updates via email or social media?*
  • Do you agree to update your contact information should you relocate in the future?*
  • Do you have a planned sitter/boarding if you are to go out of town last minute? If you are local I always take my babies in to watch after they go home.*
  • Do you have a veterinarian you plan to use?*
  • My contract states that for the guarantee to be held for the full year they need to be examined within 3 days of bringing your new puppy home. Will this be doable? If not, guarantee will be void. Do you understand?*
