Should your dog(s) require immediate emergency care due to life threatening or urgent conditions they will be taken to Black Forest Veterinary Clinic at 12655 Black Forest Rd Suite 118, Colorado Springs, CO 80908 or to Southern Rockies Animal Emergency + Specialty at 5886 Tutt Blvd, Colorado Springs, CO 80923. We will contact you as soon as possible, and will contact an Emergency Contact(s) if needed. You will be contacted to discuss non-life threatening ailments and injuries before treatment is sought. In event of life threatening emergency or when immediate veterinary care is needed, if we cannot reach you we will still have a veterinarian begin treatment.
I authorize life-saving care to be given by a licensed veterinarian not exceeding the below amount to begin diagnostics and treatments in case of immediate emergency while Black Forest Dog Resort staff attempts to reach me and/or my emergency contacts. I understand Black Forest Dog Resort staff will contact me as soon as possible, typically before leaving for the vet or while my dog is en route to the vet, and they will approve diagnostics and treatments that the veterinarian(s) believe to be in my dog's best interest even if I or my emergency contacts cannot be reached. Any cost incurred by Black Forest Dog Resort or other persons to treat my dog will be reimbursed within fifteen days of receipt.
