Declaration
I confirm that Shrewsbury School is my first choice of school for my child. I hereby accept the conditional place which has been offered for my child (named above).
1. Conditional place: I confirm that the offer of a place by the School is conditional upon my child meeting the applicable entrance requirements of the School. I am aware that the offer of a place by the School will be withdrawn and the Acceptance Deposit refunded if my child does not satisfy the entrance requirements. The deposit will not be refunded if the child is withdrawn for any other reason.
2. Current school: I confirm that fees payable to my child's current and any previous schools have been paid or will be paid in full before my child enters the School. Except as disclosed in a confidential letter attached to this form, my child has not been withdrawn from or asked to leave another school as a result of misconduct and is not under investigation and has not been convicted for any criminal offence.
3. School fees: I understand that the School may make enquiries of my child's current or previous schools for confirmation that all sums due and owing to such school(s) have been paid and I also understand that the School may inform any other school or educational establishments to which I propose sending my child if any fees of this School are unpaid.
4. Confirmation of declarations: I confirm that the declarations made on this Acceptance Form are true and that I have disclosed all information required in the declarations. I understand and agree that the School has the right to terminate this contract for educational services immediately if any declaration is found to be untrue.
