Faculty Fellowship Review-Chair Report

Reviews due by midnight on Monday, September 29th, 2025.

Prepare a separate evaluation form for each fellowship application. Please note that you cannot save the information, so you should be prepared to complete the form in a single sitting. You do not have to do all of the forms at one time.

Responses to all items are required to submit the form. When you finish the form, please click submit to send the information to the Office of Research. If you do not receive a thank you pop-up, your report was not received. If you missed the pop-up, you should also receive an email, although these sometimes end up in spam filters.
