Pizza Lunch Order Form
SECTION I: ORDER INFORMATION AND FORM
The 2025 Catholic Schools Week Pizza Lunch will be held on Wednesday, January 29. Advance orders for pizza lunches must be placed via this online form. Cheese pizza only is being provided for the lunch.

In order to ensure that all families are accounted for before the pizza order for the day is placed and that adequate seating is available at all lunch shifts on the day of the Pizza Lunch, families are asked to submit an online pizza lunch order form even if children and adults will bring their own cold lunches or if students/families will be unable to attend.

If no pizza lunch option is desired, simply select the "Cold Lunch" option in the Order section below and then send a lunch with your child(ren) to school on January 29.

NO HOT LUNCH WILL BE SERVED ON JANUARY 29.

If your family will be unable to attend the Pizza Lunch event, simply select the "Not Attending" option after providing family name and contact email information. Then select "Not Attending/No Charge" in the Payment Option section to submit your "order."

All online orders for the CSW pizza lunch must be submitted by Wednesday, January 15. Payments (payable to SVdP if by check) must be received by the school office by Wednesday, January 15, as well. If paying by check or cash, please send payments in envelopes marked with at least one student's name so that payments can be matched to participants (this is especially necessary for cash payments).

Online payments via eFunds for School ARE being accepted for the pizza lunch; standard convenience fee charges will apply. After completing your order, you will be able to select your payment method. If using eFunds for Schools, you will log in to your eFunds account, select the Pay for 2025 Pizza Lunch Orders option, enter the amount due in the Pizza Lunch Orders data window, and click on the Add to Cart button. Payment processing will then proceed as usual.

DO NOT process Pizza Lunch payments as a deposit to a child's regular lunch account. Use only the Pay for 2025 Pizza Lunch Orders option.

PLEASE NOTE: If you are unable to attend the Pizza Lunch after signing up and paying OR if the pizza lunch is rescheduled due to a snow day and you are unable to attend on the new date, NO REFUNDS will be made.

MEAL OPTIONS

COLD LUNCH: Affected students and guests will provide their own lunches and beverages from home with no payment due for affected individuals. We ask that parents not place online orders for delivery to the school; volunteer staff working the Pizza Lunch will not have time to deal with delivery people.

PIZZA LUNCH: $6.00 for 2 slices of pizza, a chocolate chip cookie, and one milk or small bottled water

LUNCH TIMES

Child Care and Grades K & 1 begins at 10:50am
Grades 2 & 4 begins at 11:30am
Grades 3 & 5 begins at 12:10pm
Grades 6, 7, & 8 begins at 12:45pm
(Grade 6 normally eats earlier but will have lunch at 12:45pm on the
pizza lunch day)

LUNCH PROCEDURES

Everyone who orders a pizza option will be given a ticket the day of the pizza
lunch. Students will receive tickets from their homeroom teachers; the options selected in the Homeroom fields below will determine where student tickets are delivered. Parents/visitors may pick up their tickets in the upper commons prior to lunch. Tickets must be presented at lunch in order to receive the ordered meals.

Again this year, visitors will be asked to meet their child outside his/her
classroom door five minutes before the scheduled lunchtime. It will not be necessary to report to the front office for check-in for this particular event.
ORDERING INFORMATION

In the "Student" sections below, provide order information for all of your children in grades K-8 or who will be in Child Care over the affected lunch times. Order information for all other attendees (parents, grandparents, siblings not attending SVdP, SVdP staff members etc.) should be listed in the "Visitor" sections. If you enter a name in any of the entry windows provided for that purpose, you will be required to also enter Homeroom or Lunch Time and Order information. Leave any unneeded entry windows blank.

If some or all of your family members will be taking the "Cold Lunch" option and not ordering pizza, please complete the appropriate "Student" and "Visitor" lines below and select "Cold Lunch" as the Order option. Again, this will help us account for all students and families and provide adequate seating for all lunch shifts.

The charge for each meal order will be automatically calculated for the appropriate field.

AMOUNT DUE INFORMATION

Sub-total amounts for child and adult meals will be automatically calculated below based on your entries in the preceding section.

If you wish to make an optional donation to the fund for ongoing Teacher Appreciation efforts, please enter the amount of that donation in the appropriate entry box.

The total amount due will be automatically calculated based on the information
you provide for student/visitor meals and bottles of donation amount (if any). You will receive an email summary of your order.

As noted above, please pay the amount due no later than Wednesday, January 15. If paying by check (payable to SVdP) or cash, place all payments in an envelope marked Pizza Lunch with at least one child's name and send to the school office.

SECTION III: ONLINE PAYMENT
If you are paying for your pizza lunch order online:

2. Click on the Submit button below. Your order will be transferred to the appropriate personnel for processing. You will be re-drected to SVdP's eFunds for Schools payment portal. Once logged into your account, select the Pay for 2025 Pizza Lunch Orders option and enter the amount shown above. Then process your payment as usual.

3. You will receive an email confirmation of your pizza lunch order after submitting the order. Please review that information; if anything was entered and submitted in error, contact Ray Bentzen (bentzenr@svdpomaha.org) with the corrections. Please note that the confirmation email is not a receipt for payment; you must still log in to your efunds account and process the payment for the amount due.

Orders are not considered final until payment in full has been received.

SECTION IV: PAYMENT BY CHECK/CASH
If you are paying for your pizza lunch order by check or cash:

1. The amount due for your order is: ${totalAmount}.

2. Click on the Submit button below. Your order will be transferred to the appropriate personnel for processing. You will be redirected to SVdP's eFunds for Schools online payment portal, but can exit your browser at that point as you are paying by check or cash.

3. You will receive an email confirmation of your order after submitting the order. Please review the information on that confirmation message; if anything was entered and submitted in error, contact Ray Bentzen (bentzenr@svdpomaha.org) with the corrections.

4. Please send your payment to the school office in an envelope marked Pizza Lunch and including the name of at least one of your children (this is especially important if you are paying with cash).

Orders are not considered final until payment in full has been received.

SECTION V: NOT ATTENDING OR NO CHARGE
