Grant Application 2025

This is a multi-page application form.  Please read the following requirements related to documentation needs and have the information available in PDF format while completing the application. You'll have the opportunity to upload relevant documentation later in this application. Please name these documents appropriately to the content and your organization name. 

REQUIRED DOCUMENTS:

Applications submitted without these documents will not be considered.

1. Most recent IRS 501(c)(3) tax-exempt determination letter. 

2. Compiled financial statements from the two most recent fiscal years or the most current 990 form; all of these documents must be dated and signed.

3. Annual report, if available. (Optional)

4. A list of the Board of Directors of the organization, including the primary business affiliation of each. You may use an existing document.

5. A complete list of corporations and foundations that have given to the organization in the previous year and how much each has given.

6. Annual operating budget for the current year.

7. Program/project budget for which you are requesting funding. Please be as specific as possible. Include any funds raised to date and the origin of said funding.

8. A signed letter of support for your project or for the organization and this funding request. This could come from a board member, volunteer, local official, or beneficiary of your organization. This is a recommendation or testimony to support your request.

9. Completed responses to the application questions in Section II on the next page. Failure to conform to word count limitation could cause an application to be rejected.

10. Your logo in a high-resolution image format.

OUR MISSION:

The West Valley Mavericks Foundation's mission is to assist families, children, and others in need and to improve the quality of life specifically in the West Valley communities through donations to Arizona based organizations. Our organization also grants awards to beneficiaries annually with funding amounts changing annually.

DEADLINE:

The grant process for 2025 is open from November 25, 2024 to January 15, 2025. The funding application and necessary attachments must be submitted online. Notification will occur in the first quarter of 2025. A charitable organization may only apply once per year and must also reapply in subsequent years for additional funding.

SECTION I

ELIGIBILITY AND GUIDELINES:

To be eligible, a charity must hold a current 501(c)(3) non-profit status and be active in Arizona. The West Valley Mavericks Foundation reserves the right to accept or reject any application and to approve or disapprove any proposal for funding, including for incomplete applications. The West Valley Mavericks Foundation also reserves the right to change the application criteria at any time. Please refer to the list below for funding guidelines. Other guidelines may apply and all applications will be evaluated on an individual basis.

In order to be considered for a grant greater than $1,000, the nonprofit organization's 501(c)(3) status must have been in operation for more than three years. 

CRITERIA:

The West Valley Mavericks Foundation does NOT provide funding to:

• Individuals,
• Political, labor or fraternal organizations,
• Funding advocacy or lobbying activities for issue-related topics,
• Funding medical or scientific research,
• Endowments, fellowships or individual scholarships,
• Religious organizations, churches, or programs that are purely denominational in purpose,
• Foundations or organizations, which are themselves grant-making entities,
• Individual schools and their support organizations (including Booster Clubs, PTA/PTO’s, athletic teams, bands, performing arts groups, alumni associations or reunions),
• Funding debt reduction campaigns,
• Funding for general operating needs or ongoing support,
• Funding for projects and/or programs outside of Maricopa County, unless the endeavor serves primarily Maricopa County residents,
• Organizations that discriminate on the basis of race, color, gender, national origin, marital status, age, disability, or veteran status.
