This is a multi-page application form. Please read the following requirements related to documentation needs and have the information available in PDF format while completing the application. You'll have the opportunity to upload relevant documentation later in this application. Please name these documents appropriately to the content and your organization name.
REQUIRED DOCUMENTS:
Applications submitted without these documents will not be considered.
1. Most recent IRS 501(c)(3) tax-exempt determination letter.
2. Compiled financial statements from the two most recent fiscal years or the most current 990 form; all of these documents must be dated and signed.
3. Annual report, if available. (Optional)
4. A list of the Board of Directors of the organization, including the primary business affiliation of each. You may use an existing document.
5. A complete list of corporations and foundations that have given to the organization in the previous year and how much each has given.
6. Annual operating budget for the current year.
7. Program/project budget for which you are requesting funding. Please be as specific as possible. Include any funds raised to date and the origin of said funding.
8. A signed letter of support for your project or for the organization and this funding request. This could come from a board member, volunteer, local official, or beneficiary of your organization. This is a recommendation or testimony to support your request.
9. Completed responses to the application questions in Section II on the next page. Failure to conform to word count limitation could cause an application to be rejected.
10. Your logo in a high-resolution image format.
