PCC General Permission Slip
My chorister is in my legal custody. I have full authority to give this permission. They have my permission to participate in all Phoenix Children's Chorus programs and activities including rehearsals, performances, events, camp and tour conducted and/or sponsored by Phoenix Children's Chorus. I also agree to allow PCC to photograph and publish photos of my child for purposes of marketing and advertising PCC to the public.
Approval of Medical Attention
In case of sickness or accident, I give permission for medical attention and the administration of medication and treatment as prescribed by the chorister's physician or as determined by an available physician, nurse, health professional or first aider.*

Please note: For touring choirs, we take medical personnel with us for the safety of the choristers, chaperones, and staff. If you select no, please be aware that a conversation must be had with the staff and medical team in order to understand your wishes while also ensuring that we can make safe and emergent decisions while on tour.

PCC Waiver & Release
Waiver of All Claims, Release from Liability, Assumption of Risk, and Acknowledgment

My chorister will be a participant in the Phoenix Children's Chorus choir season including but not limited to Williams choir camp, rehearsals, performances, events and tour. In consideration of being permitted to participate in said choir events, I the undersigned in full recognition and appreciation of the dangers and hazards inherent in domestic and international travel to which I may be exposed during my participation in said event, do hereby agree for myself, my heirs, and personal representative(s) hereby release and hold harmless Phoenix Children's Chorus, Phoenix Bible Church, Lost Canyon Camp, the City of Phoenix, Highland Lakes School, Accent Concerts, Tempe Center for the Arts, City of Tempe, and all its officers, agents, employees, and volunteers from and against any and all claims, demands and actions, or causes of action, on account of damage to personal property or personal injury or death which may result from my participation, and which result from causes beyond the control of, and without the fault of negligence of the pervious stated agencies, its officers, agents, employees, or volunteers during the period of my participation as aforesaid.

I, the undersigned further acknowledge and understand to reimburse and hold harmless the above mentioned organizations for any financial liability or obligations for injury and damage to the person or property of others caused by my actions and I, the undersigned further acknowledge and understand that the Phoenix Children's Chorus has full authority to terminate my participation in choir for any failure to comply with the rules, standards, and instruction of the Phoenix Children's Chorus with no refund of fees, and that in such event the chorister may be sent home at the expense of the undersigned.
