For the most up-to-date and accurate information regarding your specific situation, we recommend that you inquire about network status, coverage, and benefits directly with your health insurance provider. You can typically do this by calling the member service number found on the back of your insurance card or checking your plan's online portal.

Traditional Medicare — Part B*

*We do not have contracts with any Medicare Advantage [Part C] Plans but are typically willing to bill your Medicare Advantage plan for covered services, although we are not contractually obligated to do so. If you have Medicare Supplement Insurance [Medigap], your Medicare Part B claim will automatically be forwarded to your Supplement plan for adjudication.

Medicaid — Wisconsin ForwardHealth**

**We do not have contracts with any privately managed Medicaid plans and are thus unable to receive payment for services provided to individuals with Wisconsin Medicaid coverage that is managed by a private insurer, nor can we legally accept direct, out-of-network payments from individuals who are covered under these plans.

If you do not have health insurance, prefer not to use your health insurance, or have a health insurance plan that is out-of-network with our practice, we offer transparent, direct pay, fee-for-service pricing upon request as well as as our innovative Integrative Skin Health & Longevity Membership option.
