CLASS SELECTION

Please see below and select your classes you would like to enrol in. Please refer to our weekly program before selecting your classes. Classes on our program indicate the appropriate age an athlete should be to join each class.

HDSAF Competition Squad Athletes: It is compulsory for all competition squad to enrol in 3 classes minimum (age 5-12) and 5 classes minimum (age 13+) each week in 2025. To make it easier for our HDSAF Competition Squad athletes, there is now 3 choices of membership available. These memberships are great value, so please choose one membership option. In addition to classes for competition squad it is compulsory to book in 2 minimum SGT semi-privates per term. These can be arranged with coaches-No need to enrol in SGT on this form, this is communicated via our members facebook page.

PAYMENT OPTIONS

2. HDSAF Competitive Squad member Fee $220 All ages
3. EPS Development Squad Member Fee $30 
4. EPS School Squad Member Fee $100

This is a yearly fee, paid per year. No refunds will be given after payment is received. Participants will not be allowed to join class without this form and payment submitted.

Assumption of risk
I am the participant (over age of 18) or parent, carer of participant (under the age of 18) named on this document. I recognise that minor and or major injuries can include strains, sprains, broken bones, or other more serious injury in dance, acrobatic and fitness activities. I clarify that I deem my child competent and fit to participate in all activities associated with the classes/disciplines I have selected for their participation, and I accept ALL risks associated with their participation.

Release of liability
In consideration of myself, or my child(ren)s participation, I hereby for myself and my child(ren), and our respective heirs and successors, promise NOT to sue and forever release and discharge Harriman DSAF, it's coaches, directors, employee and volunteers, from all liability resulting from damages or injuries incurred as a result of participation in Harriman DSAF activities, programs and competitions, including those resulting from acts of negligence without permission.

Medical Emergencies
In case of an emergency, I hereby give consent to certified Harriman DSAF coaches and/or health professionals to administer emergency first aid to the athlete and/or obtain medical and/or ambulance assistance as required. I agree to pay any costs incurred in relation to this.

Charges and class schedule changes
Harriman DSAF reserves the rights to increase fees and charges with 30 days notice. Class times and days are not likely to change although HDSAF reserves the rights to move classes for reasons of large numbers in a class and/or not the minimum number in a class for that class to run. Changes will normally come into effect commencement of the following term.

Enrolment and Payment Conditions
Enrolment commences on the date started on the enrolment agreement. Registration fee is to be paid before the beginning of the first enrolled lesson. Class fees are to be paid within the first two weeks of the commencement of each term. Invoices will be sent to your nominated email address once enrolments have been submitted. Fees are calculated on classes nominated and are charged regardless of attendance. Discounted Fees of 10% only applies to families. Discounted fee will be applied to fee payable of the 2nd enrolled member only and again with additional family members enrolled. First enrolled no discount. Excludes Small Group Training, Private Lessons, EPS School & Development Squads, National squad training & Holiday programs.
Classes will still operate on Public Holidays, for most of the year it is especially important for squad to continue to train on these days. Clarification will be sent out of this information prior to the day (this information is important to competition enrolled members). Classes do not run in school holidays our yearly classes run in 4 terms in step with public school terms. If classes are offered in school holidays, Sundays and/or extra training days scheduled a new invoice will be sent out prior to these lesson dates.

Cancellation
Cancellation of a class needs to be put in writing and emailed to harriman.dsaf@outlook.com with 2 WEEKS NOTICE. If you fail to do so, your class enrolment will not be cancelled and classes will continue until received written confirmation.

Refunds
All payments, including deposits are non-refundable. Refunds for change of mind or missed lessons due to minor sickness, short holidays or minor injuries are not permitted. Refunds will only be approved for long term travel and serious sickness or injury. Travel of more than 21 days (3 weeks) during term dates and provided a doctors /specialists certificate is received for long term recovery from injuries and sickness. Most cases the athlete/dancer is still able to attend classes and work on other areas of their training, coaches are always willing and able to provide options in these circumstances.

Missed lessons
Make up lessons are not guaranteed but may be available by filling in a make up lesson form on the HDSAF website. If you miss a lesson a make up lesson can be offered, per participant during the same term. Please do not email through your make up option date and class, we will only accept forms submitted through our website. Make up lessons can be any class in your age group, on any day and any location. Please only attend your make up lesson with approval from our team, there may not be available spots in the class you have requested. 

Serious sickness, injury or long periods of overseas travel
Harriman DSAF will only credit accounts and/or refund term classes and charges with written notice, we prefer as much notice as possible. Supporting evidence relating to your request include medical certificates from your specialist and travel dates of more than 21 days.

Parking Policy
Ermington Studio ONLY
It is important that all parents and members park outside of the school grounds at Ermington PS. Parking available on Brush rd. Please only enter through school gate on Brush Road to access hall. Strictly no parking or driving onto the school grounds.
Both studio locations
Please make sure each child is accompanied by an older person and dropped to the door of both premises, ready for class. Please don't allow your child to leave the building and/or school unaccompanied. A coach will not allow a child to walk home by themselves.

Safety and Sickness Policy
SAFE PROCEDURES
I will agree and abide by the policies and procedures HDSAF have in place during their classes. Parents and caretakers to be outside of studios at all times and only allowed to view classes on special events and open days set by HDSAF. I will keep myself and/or my participant home if unwell at all times.

Media Permissions
I am aware that individual and group photos and videos are taken throughout programs/activities whilst at HDSAF, I hereby grant permission for my or my child(ren)s images to be used in publicity, media usage including but not limited to social media, website, training tools and advertising on behalf of Harriman DSAF.

Inaccurate Contact Details
I understand that if I do not provide accurate and up-to-date contact information I may miss out on important and timely information which may affect my enrolment or result in missed opportunities.

Participation
I confirm that the information set out on this form on behalf of the athlete/dancer is true and correct to the best of my knowledge and is being relied upon by HARRIMAN DSAF and its staff. I know of no reason or fact affecting the members health which would affect or preclude him/her from physical activities.
