Please complete this application to nominate a student for the Conway Nursing Pathway Program.
You may nominate a person who is actively pursuing or planning to pursue a BSN degree in nursing or a nursing student who has completed or will be completing an associate degree in the next semester.

In addition to responding to application questions, the person nominating the student must have the following items available to upload in order to submit the application.

- 2 references (one academic and one professional)
- Student Essay
- School Transcripts
- Professional Headshot (if available)

Recipients of the Conway Nursing Pathway Program will be required to agree to program requirements associated with opportunities for career preparation and advancement, including summer work, scholarships, and one-on-one nurse mentoring relationships.
Additional program requirements and expectations will be provided to all candidates who are selected for participation in the program.
