By submitting this request, you agree that if this reservation is confirmed, you hereby agree to accept and abide by the Blessed Sacrament Catholic Church Facility Use Agreement. The terms and conditions become a part of the official Facility Use Agreement to be signed by all parties.
Blessed Sacrament Catholic Church Facility Use Agreement
It is the group/organization’s responsibility to notify Blessed Sacrament Catholic Church (BSCC) Parish Office of cancellations or changes to this request no later than 10 days prior to the event. Failure to do so will result in forfeiture of all deposits. Rescheduling requests or change to dates and times are not guaranteed and are at the discretion of Blessed Sacrament Catholic Church. Church Liturgical/Specific Events take precedence.
Parking requirements must be specifically identified in this agreement. Due to potential BSCC conflicts (i.e. Mass), user parking may be restricted. If special parking requirements are needed (i.e. bus parking), they must be also specified in this agreement in writing a minimum of 5 days in advance.
I. Facility Usage Agreement
The User agrees to comply with the following terms and conditions:
- The User must pay the full, agreed upon amount per this agreement for the use of the facility. All monies, including deposits must be paid in full and received by the BSCC Office to validate this agreement and before any reservations are confirmed.
- The User must reimburse BSCC for all damages caused to the facilities and/or the equipment therein.
- The User assumes all responsibility and liability for the requested use and agrees to defend, indemnify and hold BSCC harmless from all loss, damage or injury to any person or property arising from use of the facilities.
- Insurance
- If any Diocesan property is rented or provided free of charge for gatherings the individual or organization is required to provide a certificate of insurance as requested by the Diocese Policy “Insurance Requirements for Contractors, Service Providers and Vendors”.
- If the individual or organization cannot provide the acceptable certificate of insurance, Facility Use Insurance must be purchased by the individual or organization. The website for the event insurance is https://www.kandkinsurance.com/programs/event-insurance/catholic-diocese-tulip-insurance
- Once coverage is purchased online, the person submitting the event request will receive a copy of the purchase summary (the application), an evidence of coverage certificate and a certificate naming Diocese of Pensacola-Tallahassee, Bishop William Wack, Trinity Catholic School, and Blessed Sacrament Catholic Church as additional insured. A copy of this certificate must be provided to the BSCC Office no later than 10 days prior to the event.
- User may also purchase coverage for vendors during the same transaction. Example: Use of a DJ at a party that does not have insurance coverage.
- The User agrees to reimburse BSCC for any damage, clean-up or hazardous waste related expenses, including disposal, shipping, damages, and or penalties imposed because of its use.
- The parties agree that the User, its agents and employees are not agents or employees of Blessed Sacrament Catholic Church. The User will be solely responsible and will assume full responsibility for the actions and omissions of its guests, agents and employees when using BSCC equipment or on the property. User is solely responsible for worker's compensation coverage of its agents and employees.
- The User will be solely responsible to provide adequate supervision of the activities conducted in BSCC facilities and property.
- The User will comply with all applicable state and federal laws as well as Blessed Sacrament Catholic Church and Diocese of Pensacola-Tallahassee regulations and requirements.
- Disorder or disruption of BSCC activities will not be tolerated. Persons attempting to endanger the safety of children, personnel or other adults, or damage to any property shall be held accountable and required to leave the premises immediately.
- Weapons are prohibited.
- Pets (except designated service animals) are prohibited.
Smoking and the use of any tobacco products is prohibited.
- All alcoholic beverages, unless specified in the agreement and covered by insurance, are prohibited.
- The selling of alcoholic beverages is prohibited.
- BSCC reserves the right to require a staff member to be on site during the event. If required, additional charges may apply to the User.
II. Facility Use Guidelines
The User agrees to comply with the following terms and conditions:
- Facility Availability
- The use of the facility is subject to availability and the discretion of Blessed Sacrament Catholic Church. The Blessed Sacrament Catholic Church reserves the right to decline facility usage requests at their discretion.
- In the event of scheduling conflicts, the group/organization representative, is responsible for working with BSCC to find reasonable alternate spaces in a timely manner. In the event of a scheduling conflict, the User will be notified no less than one week before the conflicting date.
- Capacity of rooms used shall not exceed those agreed upon.
- Facility Setup
- All room setups are the responsibility of the User unless other arrangements have been made with BSCC.
- Chairs, tables, and other furniture are not to be moved from one room to another. Under no circumstances is the Altar to be moved in or outside of the Chapel. All equipment, setup of furniture, etc. must be rearranged according to BSCC preference and returned per BSCC layout at the end of the event by the User unless otherwise agreed to in writing. Final layout request will be provided a minimum of 1 day prior to event start. Use of BSCC furniture, equipment, etc. is subject to additional charges.
- The User must coordinate with BSCC to adjust room temperature prior to the start of the event. If temperature needs adjustment during the event, please consult with BSCC.
- Any windows and doors opened during the event must be closed and locked upon conclusion of the event or whenever the group leaves the facility.
- Facility Protocol
- Access to any facility or property will be provided by BSCC based on the agreed upon schedule. BSCC is not responsible for meeting with suppliers or vendor. User must be available for all suppliers or vendor meetings, questions, etc.
- All doors are to be locked each night. All windows are also to be shut and locked.
- All activities must conclude per the agreement including the cleaning and locking of the facility.
- If using the kitchen, the User is responsible for making sure the stove, coffee maker, and any other appliances are turned off and properly cleaned at the end of the event.
- All items, including food, paper supplies, etc., brought to the premises must be removed at the end of the event unless prior agreement with BSCC is made in writing in advance.
- Used areas, restrooms, hallways, etc. lights must be turned off at the end of the event.
- The User must report any maintenance issues on or around the premise to BSCC as soon as possible.
- Children are not to be left unsupervised on the premises.
- Unless specific permission has been granted, groups are not permitted to use any Blessed Sacrament Catholic Church, Trinity Catholic School, or Diocese of Pensacola-Tallahassee facilities or properties not specifically included in this agreement.
- Exiting
- All dining/cooking items must be washed, dried, and returned to the appropriate area before departing.
- Kitchen sinks and counters must be cleaned thoroughly, and the floor swept.
- Any waste left from the event must be taken to the designated, appropriate outside trash receptacles and new liners placed in the cans.
- The User must clean all spills immediately using clear water and paper towels. If additional cleaning supplies are required, please notify BSCC.
- The User may use the refrigerators in the kitchen to store items for the event. Access to store items on the property may occur per the date and times of this agreement.
- The User will be responsible for removing all items (perishable or nonperishable) brought onto the property no later than the end of the event.
- Usage agreement includes use of restrooms. All restrooms must be left in clean condition (sinks cleaned, trash removed, toilets flushed, etc.).
- Failure to abide by the above conditions may result in the loss of privilege of using Blessed Sacrament Parish facilities in the future, forfeiture of deposit and/or other additional charges.
- Parking:
- User will have use of the Parish parking areas unless specified elsewhere.
- All state and federal laws apply to parking on Parish property. All parking must comply with signage and only in designated parking areas. Illegal parking is subject to towing at the car owner’s expense.
- Parking behind the Neumann Center, in fire lanes, no parking areas, exits and driveways is prohibited.
III. Schedule of Deposit and Usage Fees
- Deposits:
- Deposits are refundable providing all activities are performed as agreed upon per this agreement.
- Deposits are not refundable if the User cancels the event less than 10 days prior to the event.
- Failure to use all property per the agreement (i.e. cleanliness, trash removal, etc.) will forfeit the deposit and may result in additional charges.
- Access or use of the Kitchen is NOT included in in this request, unless specifically covered in this agreement. (Any use of kitchen facilities and pricing must be signed by NC Director and attached to this agreement.)
- Access or use of any property, space or room(s) not listed per this agreement is prohibited and may result in additional charges and/or forfeit of deposit.
- Access to restrooms is included in this agreement.
- Deposits may be refunded if all conditions of the agreement have been met. Deposit refunds will be processed no later than 10 days after the last day of the event.
- Deposits and/or fee usage charges are negotiable at the discretion of the Pastor and Parish Administrator.
- The Neumann Center does not require a deposit but charges a daily fee of $100 for use of the community room or dining room only and $150 with the addition of the dining room and full kitchen.
