I understand that this booking request form does not guarantee me an appointment. I understand that I will be contacted for follow up via email or direct message within 48 hours, and will be required to submit a deposit via cash, CashApp, Venmo, or Zelle before an appointment is confirmed. I understand that this is a non refundable deposit. If I need to reschedule my appointment I will do this no later than 48 hours before my appointment to avoid losing my deposit.
I understand that I must be 18 or over to have a tattoo placed and must bring a government-issued photo ID to my appointment, whether or not I have been tattooed at Average Joes’ Tattoo Shop before.
