This form is for recognizing outstanding volunteers. Each year, we honor four volunteers: three Member of the Year and one Volunteer of the Year (usually a hearing person. You can fill out this form in writing. If needed, you can also submit an ASL video to explain your answers. Please fill out as much as you can.

You can submit this form as many times as you’d like, but only one nominee per submission.

- The more details you can share, the better we can understand the impact they’ve made.
