In accordance with Code of Ordinances Section 4-12.3, all dogs and cats in Fayette County must be licensed annually starting at six months of age. A spayed or neutered animal qualifies for an $8 altered license. An animal that has not been spayed or neutered is required to have a $40 unaltered license. Proof of rabies vaccination is required for ALL licenses, and spay/neuter proof is required for altered licenses. For each online license, $2 is added to the license fee for processing and postage (to send license). PAYMENT: Payment of $8 (altered animal) or $40 (unaltered animal) per pet, plus $2 online processing fee, must be made in order to complete and submit this application.
RABIES VACCINATION PROOF: You must include proof of current rabies vaccination certificate or veterinary receipt by uploading it at the bottom of this form for each pet you are registering for.
SPAY/NEUTER PROOF: To obtain an altered license where the pet's information is not already on file with LFACC, you must include proof of pet spay or neuter surgery (i.e. surgery record, veterinary receipt) by uploading it at the bottom of this form.
