By signing this form, you agree to the following conditions:

1. Consent for Use: I give Radical Elevation permission to use my/my child's image, photograph, video recording, and/or audio recording for the purposes of public relations, marketing, education, or outreach.
2. Media Formats: I understand that the media may be used in print, electronic, video, or other digital formats and may be shared publicly.
3. No Compensation or Ownership: I understand that I will not receive financial compensation or royalties for the use of these materials, and I waive any ownership rights over the content.
4. Possible Recognition: While identifying information such as my name will not be included unless I give specific written permission, I understand it is possible that someone may recognize me.
5. Edits and Alterations: I consent to the media being digitally edited (e.g., for size, coloration, or background changes), provided that the edits do not change the intended meaning or purpose of the content.
6. Voluntary Participation: I acknowledge that my participation is voluntary and that I may revoke my consent at any time by submitting a written request to Radical Elevation, except to the extent that Radical Elevation has already acted on this consent or released the materials into the public domain

By signing below, I acknowledge that I have read this release form, fully understand its terms, and agree to them voluntarily.
