1. All dogs and cats must maintain current Rabies vaccination status as required by Massachusetts law.
2. While at the clinic, all dogs must have properly fitting collars or harnesses attached to a leash at all times (no retractable leashes accepted) and cats or exotic pets must be in secure carriers.
3. If you clicked 'yes' on the chill protocol, you will be scheduled for a Meet and Greet visit with one of our technicians to assess behavior prior to a Dr visit.
4. We are committed to maintaining a respectful and professional environment for our clients and patients and ask all clients and visitors to do the same. Any aggressive or threatening behaviors will not be tolerated and will result in immediate termination of services.
5. You agree to the above professional conduct standards.
6. Payment is due in full at time of service via Cash, Credit/Debit card or Care Credit.
7. By signing this form you agree to allow Hillside Veterinary Clinic to provide medical services to your pet.
8. Clients will be liable for any and all collection costs related to unpaid invoices and debt recovery. Any unpaid invoices will result in immediate termination of services and will be sent to collections.
9. Cancellations must be made at least 48 hours before the appointment. Late cancellations and no show appointments will be charged a cancellation fee equal to an exam. This must be paid prior to any further service provided.
10. You agree to allow Hillside Veterinary Clinic and its associates to contact prior medical facilities for obtainment of records.
11. You agree to a booking fee of $50.00 that will be applied as a credit at your first visit. Full medical records must also be received prior to booking.
12. Signing this agreement is explicit acceptance of all above terms and conditions. The signatures executed electronically are legally valid and enforceable.
