Personal Training Agreement & Liability Waiver

In full consideration of the risk of injury while participating in the Activity, and for the right to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily participate in this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activity, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any kind of risks related to traveling to and from as well as participating the Activity, which may include, but are not limited to, physical or phycological injury, pain, suffering, illness disfigurement, temporary or permanent disability, economic or emotional loss, and death.

I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage. 

Terms of Agreement

This Course will automatically bill monthly on the anniversary date of your enrollment. The House Of Warriors is authorized to charge the account whose information you provide here to fulfill your obligation to pay tuition. The following fees may be applied to your account, over 10 days late $10. No Book Fee $15, No Show Fee $15

It is intended and agreed that all tuition payments are NONREFUNDABLE. You may cancel this agreement without if,

you relocate your residence further than 25 miles from any school operated by The House Of Warriors, Have a medical condition preventing physical training for 6 months or greater (with doctors note) or if The House Of Warriors is out of operation for greater than 30 days.

All other cancels require a 30 day notice. You must request your cancelation form by notifying the studio (Text or Email). One will be sent and must be turned in to process your cancellation. Your training will be terminated 30 days after it is processed. (If the request is put in 2 days before billing, you will be charged for the next month's tuition) Once the cancellation is processed, you will no longer be charged for services. Any services charged may still be used until end of term.

FREEZE POLICY: In the event of being on an extended leave, longer than 2 weeks, members will have the option to freeze their account for $25. This preserves your spot (and current pricing plan) in class and will not require a re-enrollment. During a freeze period, members will be allowed to attend one or two times during that freeze month. The maximum amount of time allowed to freeze is no more than 2 months

Class Bookings: It is agreed that members and or responsible parties must pre-book their intended classes before attending. (through use of the studio app or class booking website) The minimum time allowed to book a class or cancel is 1 hour before start time. All participants must be on the daily training list to participate.

If a member is not on the training list, If space permits, you will be allowed to enter and be charged a $15 no book fee. If a member is booked and does not show with out a proper cancellation, a $15 no show fee will be assessed.

Model Release Waiver
I, the undersigned, grant Master Dave's House of Warriors, its representatives, and employees the right to take photographs and/or video recordings of me and/or my child(ren) during participation in classes, events, or other activities at the Studio.

I authorize the Studio to use and publish the photographs or videos for promotional, marketing, advertising, and educational purposes, including but not limited to social media, websites, print materials, and advertisements.

I acknowledge and agree that:

I will not receive any compensation for the use of these photographs or videos.
I waive any right to inspect or approve the finished product, including written or electronic copies, wherein my likeness or my child(ren)'s likeness appears.
The Studio will not use the photographs or videos in a defamatory or exploitative manner.
This release shall remain in effect indefinitely unless I notify the Studio in writing to revoke it (cancelation of Training Agreement). Such revocation will not apply to materials already produced or published prior to the revocation.

 

Conduct: Student or reasonable party agrees to abide by all rules of the The House Of Warriors, to maintain their membership dues in a timely manner, and to wear a uniform in accordance with studio standards during training, testing and competition.

Failure to maintain valid membership renders VOID all rank guarantees made in the above named program. Uniforms must be CLEAN and in good condition. Student agrees never to use Martial Arts training in a manner that’s abusive or offensive to others in or out of the studio.

 

The House Of Warriors reserves the right to dismiss any student at any time for misconduct and/or failure to comply with the rules and regulations of The House Of Warriors.
