Photo Release Statement: As part of your volunteer activities with the Annie Catherine Pridgen Foundation (ACP), we may capture photographs, videos, or other digital media that include your likeness. These materials may be used for promotional purposes, including but not limited to ACP’s website, social media, publications, and other communication channels. Please indicate your preference regarding the use of your likeness:

I understand that these materials are the property of ACP, and I waive any rights to inspect or approve the finished product. I understand that ACP will take reasonable steps to exclude me from media content used for such purposes.
By submitting this form, and unless noted by clicking the "do not consent to use" statement above, I hereby grant permission to the Annie Catherine Pridgen Foundation (ACP) to use my image, likeness, and any photographs or videos taken during my participation in ACP activities for promotional purposes, including but not limited to fundraising campaigns, ACP events, publications, social media, and the ACP website. Additionally, I consent to the use of my name and submitted information for ACP-related activities, including but not limited to donor acknowledgment, volunteer recognition, and outreach initiatives, in accordance with ACP's mission. I understand that these materials are the property of ACP and that I will not receive any compensation for their use. I waive any right to inspect or approve the finished product or materials in which my image or information appears. Furthermore, I release ACP from any claims, damages, or liability arising from or related to the use of my image or information as described above.
