Acknowledgment of Risk
I, the undersigned, understand that participating in sports and fitness activities at The Factory Performance involves inherent risks, including but not limited to physical injury, illness, property damage, or other unforeseen events. I voluntarily assume all risks associated with these activities.
Release of Liability
In consideration of being allowed to participate in sports and fitness activities provided by The Factory Performance, I hereby release, waive, and hold harmless The Factory Performance, its employees, agents, contractors, volunteers, and affiliates from any claims, liabilities, damages, or expenses arising from or connected to my participation, except for those arising from gross negligence or intentional misconduct.
Fitness to Participate
I confirm that I am physically and medically fit to participate in the activities offered by The Factory Performance. I understand that it is my responsibility to consult a physician if I have any concerns about my ability to safely participate.
Indemnification
I agree to indemnify and hold harmless The Factory Performance, its employees, agents, and affiliates from any claims or demands arising from my actions or negligence during my participation.
Consent for Medical Treatment
In the event of an emergency, I authorize The Factory Performance to obtain medical treatment for me or my minor child. I understand that I am responsible for any costs incurred.
Photo/Video Release
I consent to the use of photos or videos taken during activities for promotional or educational purposes.
