    2026 Fee Relief Declaration and Consent Form
  • What is this form for?
    This form is for families to apply for NSW Government fee relief for eligible 3-year-old and 4-yearold children enrolled in NSW long day care services. It also gives your consent to your service to share personal information with the NSW Department of Education (the department). Please see more information about this consent on Page 3.


    What do families do?
    1. Give this completed form to the early childhood education and care service where your child is enrolled.
    2. Complete a separate form for each child enrolled at an early childhood education and care service.
    You do not need to send this form to the department.

    How much fee relief will you receive?
    Fee relief is applied after the Child Care Subsidy to reduce your child’s fees. The amount of fee relief is dependent on service provider type, location of the service, age of your child and number of hours enrolled.
    Please check with your service how much fee relief you will receive.


    Why is this form important?
    This form allows your service to apply weekly fee relief after the Child Care Subsidy to further reduce your fees.

    The service where you want fee relief to be applied. You can only nominate one service, even if your child is enrolled at multiple services

  • Note: You cannot apply for fee relief for the same child from another service where they are also enrolled. If your child is enrolled in more than one service, you must choose one service when applying for fee relief.

    The NSW Department of Education is collecting the personal information about you and your child that you put in this form.


    What information is collected?
    The department collects information such as names, birth dates, addresses, Aboriginal and/or Torres Strait Islander status, language background and service(s) enrolment information.


    Do you have to give consent?
    No, but if you don’t, you won’t receive fee relief through the department’s Start Strong for Long Day Care program.


    Why is the information collected?
    The department uses this information to run the Start Strong for Long Day Care program, including to check eligibility, manage payments, audit compliance and evaluate the program and use of funding. The information may also be used for research and developing policies related to early childhood education and care. If personal information is shared with third parties, it will only be used for those
    purposes.


    Your personal information will not be shared with others unless required by law or with your consent.


    Any published information will be anonymous and will not include names or identifying details.


    You may withdraw consent to the department’s use or disclosure of your information at any time by contacting your early childhood education and care service where your child is enrolled.


    You have the right to access and correct your personal information held by the department by writing to Legal Services – Privacy, GPO Box 33, Sydney NSW 2001, or legal.privacy@det.nsw.edu.au.
