Waivers & Photo/Video Release

INDEMNITY AND HOLD HARMLESS AGREEMENT
I agree to indemnify and hold harmless HIGH PROFILE ATHLETES INC., any employee, administrator and/or board member from actions or lack of action regarding any accident, injury, or illness, damage to your property or any other medical condition resulting in my participation in the following activity: HIGH PROFILE ATHLETES INC..  I understand that HIGH PROFILE ATHLETES INC.’s insurance does not cover any injuries sustained while participating at our location, in any outside event, or other situations not mentioned. Any and all medical claims must be filed with the participant’s “private insurance carrier” if available.  I agree to indemnify and hold HIGH PROFILE ATHLETES INC., its trustee’s officers, employees or agents harmless from any and all claims against HIGH PROFILE ATHLETES INC., its trustees, officers, employees or agents made by third parties which result from the above-named person’s actions while participating at our location, in any outside event, or other situations not mentioned.  Furthermore, I hereby expressly release and agree to hold harmless on my behalf and on behalf of the above named person, HIGH PROFILE ATHLETES INC., its trustees, officers, employees or agents from all claims or actions of whatsoever nature, in tort or in contract, that I or the above named person ever had, now has or may have in the future against HIGH PROFILE ATHLETES INC., its trustees, officers, employees or agents which result from the above named person’s participation at our location, in any outside event, or other situation not mentioned.  In consideration of the above-named person being permitted to participate in this program, I expressly waive all claims to which I may otherwise be entitled, including but not limited to, claims for medical expenses and wages.  I recognize that HIGH PROFILE ATHLETES INC., its trustees, officers, employees or agents have sovereign immunity and governmental immunity under Texas Law. I understand that HIGH PROFILE ATHLETES INC., ts trustees, officers, employees or agents are not waiving any sovereign or governmental immunity that it or they have under Texas or other applicable law. I, the undersigned, have read this release and understand all its terms. I have executed it voluntarily and with full knowledge of its significance.

RELEASE FROM LIABILITY FORM

Release from Liability Form Recognizing that HIGH PROFILE ATHLETES INC. will do its best to ensure a safe experience. I understand that the terms of this agreement shall serve as a full release and full assumption of risk to my vehicle, my property (business), myself, my heirs, executors, and administration accidents may occur in participation in youth sports activities and from transportation to and from events, therefore, I agree to assume these risks. 
By signing below, I release HIGH PROFILE ATHLETES INC., its employees, volunteers, independent contractors, directors and agents from all liability based on any damage, loss, or injury whether it is the result of ordinary negligence or otherwise, caused to my child or to me, from participation in this youth sports program. 
I have read and understand the above verbiage and have completed this form to the best of my ability. I also support HIGH PROFILE ATHLETES INC. sports philosophy, which is based on participation, fun, physical fitness, health, skill development, teamwork, fair play, family involvement, and volunteer leadership.

By signing this agreement I agree to the terms of the waiver.

PHOTO AND VIDEO RELEASE

I hereby give permission to HIGH PROFILE ATHLETES INC., to use photographs and videos taken during the games and events associated with HIGH PROFILE ATHLETES INC. in any manner to help promote the league activities. Such use could include publications, media releases, announcements, electronic or otherwise, and on league websites or social media pages. I understand that I will not receive any compensation if such image appears in any of the manners listed above or other manner that the league deems appropriate. I agree that such an image is the property of HIGH PROFILE ATHLETES INC.
