PARENT AGREEMENT

As a parent or guardian of a student attending Almadina Academy, you are expected to fulll the following responsibilities:

1. Transportation
○ Ensure the timely transportation of your child to and from school each day unless
district-provided transportation has been arranged.
○ Be available to transport your child if they must be removed from school due to illness or other circumstances.

2. School Schedule
○ Adhere to the school's schedule, including occasional cancellations of classes.
○ Follow arrival and dismissal times as outlined. Chronic absenteeism, tardiness, or late dismissals (without prior arrangements) will not be tolerated.

3. Homework Completion
○ Ensure that your child completes all daily homework assignments.

4. Volunteer Participation
○ Contribute to our vibrant school community by participating in at least 20
parent/family volunteer hours per academic year. This is a wonderful opportunity to
engage in activities such as PTO involvement, helping with classroom events,
participating in field trips, or supporting fundraising eforts. Your participation makes a meaningful difference.

5. Health Requirements
○ Ensure your child meets all immunization requirements for the State of Ohio.
○ Comply with school requirements if your child has special health needs.
○ Keep your child at home if they have a communicable disease or a temperature
exceeding 100°F.

6. Parent-Teacher Conferences
○ Participate in all scheduled parent/teacher conferences.

7. Uniform Compliance
○ Ensure your child adheres to the school’s uniform policy and attends class in full
uniform each day.

8. Acknowledgment of School Structure and Legal Requirements
○ Understand that Almadina Academy is an Islamic school established under Chapter 3314 of the Revised Code.

○ Recognize that the Academy is a charter non-public school and that students must take proficiency tests and other examinations as required by law. Additional legal requirements may also apply.
○ Be aware that students previously excused from compulsory attendance laws for home education will no longer be excused for this purpose once enrolled in a charter school.

9. Handbook Acknowledgment
○ Acknowledge that you have read and understood the handbook available on the
Academy’s website (www.almadinaacademyohio.org). Please note that the
handbook may be updated at any time without prior notice.

Tuition Acknowledgement

This letter is to acknowledge that I understand it is my responsibility to ensure that the Ohio Department of
Education receives all adequate documentation so that my child/ren receives the Edchoice scholarship.

By Signing this document I also agree to the following:
1. I understand that it is my responsibility to follow up with the Edchoice department if there are any
delays in my child/ren receiving the Edchoice scholarship.
2. In the case that my child/ren doesn't receive the scholarship, I understand that it is my responsibility to
pay the tuition in full. I also understand that, if Almadina does not receive the payment, the child/ren
may be subjected to withdrawal.
3. In the case that I withdraw my child/ren from school before receiving the scholarship, I acknowledge
that I am fully responsible for the tuition payment in the duration my child/ren was enrolled.

Enrollment Fee Acknowledgement

This letter is to acknowledge that I understand it is my responsibility to make the enrollment fee payment in full
for my child/ren at the start of the school year.

By signing this document I also agree to the following:

1. I understand that the enrollment fee is for the whole school year, not a monthly charge.
2. In the case that I withdraw my child/ren from Almadina, I understand that I am still
responsible for making the full payment of the enrollment fee.
3. I understand that if there is any lack of payment, my children will be subjected to for withdrawal from the Academy.

4. Enrollment fee is not refundable under any circumstances and listed on our website Admissions Instructions

Equipment/Supply Release Form

Equipment: Chromebook, Textbooks and any other Almadina Academy property that is deemed to be returned.

I hereby assume full financial responsibility for replacing any equipment/school material if it is lost, stolen,or damaged beyond repair. I will assume full financial responsibility for repairing the equipment and or books if it is not returned in its working condition.

I also understand that this equipment is to be used by my child/children only and only for their assigned courses for the school year.

Lastly, I understand that Almadina Academy will bill me for the current market price of item(s) that are deemed damaged or lost.

Permission for Publication and Directory Information Release

By signing below, I: Agree / Do not agree to allow the Academy to publish my child's likeness or image in any format, including print, electronic, or video media, across various platforms such as social media, newsletters, school publications, and other online channels.
Release the Academy and its representatives from any claims related to copyright ownership, publication, or compensation concerning the use of my child's image or likeness in the aforementioned formats.
