USTA ADULT SPRING LEAGUE TEAM NUMBER REQUEST FORM
IF YOU ARE REQUESTING MORE THAN ONE TEAM, PLEASE SUBMIT A SEPARATE FORM FOR EACH TEAM.

Captains should not submit a request until they have the minimum number of players ready to register.

Team Numbers will be emailed to Captains shortly after receipt of your form. Captains and players can begin registration in TennisLink on 1/1/26.

Public team captains - If you recieve your team number on or before 1/1/26, you MUST have your minimum number of players registered by 1/3/26, or the team WILL be moved to the waitlist.

Private team captains - If you receive your team number on or before 1/1/26, you must register by 1/3/26.

If you are requesting a public court with the City of Durham or the Town of Chapel Hill, please indicate your #1 court preference from the list below. Listing your preference does not guarantee all home matches will be scheduled at your requested location.
The number of available courts is in parenthesis. Do not choose a park with less courts than the number of individual lines in your chosen league level. Staggered matches are not offered.
