Leave Request Instructions
Submit this form for any time away from work. All absences or missed scheduled time must be documented using this form to ensure accurate scheduling and recordkeeping.
If you have available PTO, sick, or flex time, you must use use accrued time when requesting leave.
Failure to document time away may be subject to administrative review under organizational time-off and attendance policies.
PTO is calculated in 5-hour increments (1 day = 5 hours). Do not enter more than 5 hours per day. PTO is only paid for days you are normally scheduled to work. If the office is closed for a holiday or scheduled closure, do not request PTO those days will be paid as paid holidays. call me once
Counselors: Do not request leave in Gusto; submitting this form is the only step until approved, then block your calendar. Do Not Block your calender prior to submitting this form.
Administrative staff: Submit this form and request leave in Gusto after approval.
Notice Requirements:
Paid Time Off (PTO):
5 or more consecutive days require 30 days’ notice; fewer than 5 days require 2 weeks’ notice.
Sick Leave:
Notify your supervisor as soon as reasonably possible. A doctor’s note is required after 3 consecutive days of sick leave.
Flex Time (Counselors Only):
Flex time does not require advance notice, but the request form must be submitted. Employees are fully responsible for rearranging their schedule. Flex time is available in a maximum of 2-hour increments and is limited per quarter.
