Thank you for your interest in being a member of the ELEVATE Kentucky Class of 2026. ELEVATE is our program designed for young professionals (average age 25-35) and includes three sessions across the state. 

 

Below you will find the application. Please read the following instructions:

• All applications will be judged on their merit alone. Please take your time and be as thorough and thoughtful in your answers as possible.
• It is your responsibility to contact your references and have them provide an official reference form for you. Additional instructions are below.
• No incomplete applications (including those with missing references) will be considered.
• Other than your headshot, no attachments or other documents of any kind will be considered, including resumes, CVs, and references other than the official reference form, etc.
• Please note the tentative dates and locations of the sessions below. It is expected you attend all three sessions.
• A tuition of $2,500 will be due before starting the program.

Your application to this program is based on your merit and the recommendations of two people outside of your family who have worked with you on a personal or professional level.

Recommendations submitted in any way other than the online form will not be considered. Once you click "submit" on this application, an automatic e-mail will be sent to the e-mail address listed for each reference with instructions on how to submit their recommendation. 

Please follow up with each person listed below. Sometimes our e-mails with survey links go to junk/spam folders, and it is your responsibility to make sure your references complete the form.
