Grievances should be sent or delivered to Katie Gauthier, who has been designated as the OPRD staff member to whom a grievance may be submitted.

- Mail: 725 Summer St NE C, Salem, OR 97301 - Please include "Attn: PML Grievance" on the envelope
- In Person: At 725 Summer St NE, Suite C, Salem, OR 97301. - To submit a grievance in-person, you must submit the written notice to our front desk staff during business hours. Our office is open 8am - 5pm, Monday - Friday.
