PLEASE NOTE: All applicants aged 16 years and older are required to sign the Convention Scholarship Application. A parent/guardian's signature is also required for all performers UNDER the age of 18 Years Old.
By signing & submitting the Convention Scholarship Application, you are certifying that you have read and fully accept and understand the terms and conditions of the Scholarship Application Agreement and the Photo/Video/Image Use Waiver. In addition, you are certifying that all information is correct and that you are the person completing this Application for you or as the parent/guardian. When you press the submit button, an email copy of the Convention Scholarship Application, the Scholarship Application Agreement, and the Photo/Video/Image Use Wiaver will be sent to you.
