Dear parents/carers,

Thank you for your interest in enrolling your child on our Confirmation Preparation Course.

Before you complete the application, please note that:
1. Baptism Certificate: You will be asked to upload a copy of your child's Baptism Certificate. This field is compulsory, you will not be able to submit the application without it.
2. First Holy Communion Certificate: You will be asked to confirm the date of your child's First Holy Communion and to upload a copy of his/her First Holy Communion Certificate. If your child has not yet received FHC, then please select the most relevant category. Your child will receive First Holy Communion at the time of Confirmation.
3. Missing Certificate: If your child was baptised and/or received First Holy Communion at Christchurch, and you are unable to locate the original certificates, then please contact the Parish Office. Duplicate certificates are available for a fee of £10 cash per copy.
4. Course Fee: The course fee is £35 per child.
5. Attendance: As it is a relatively short course, your child is expected to attend every session. The sessions are not optional. Course dates and times are available on the Parish website.
  Both the child and parent/carer must attend the Introduction Evening & Group Session 1 on Wednesday 4 March at 6.30pm. Failure to attend this evening will result in your child being removed from the Course.
  It is compulsory for both the parent/carer and child to attend the Enrolment Mass on Sunday 22 March at 6.30pm. If this will not be possible, then please do not enrol your child on the Course.
6. Selection of Mass date, sponsor and Confirmation Name: These will be selected once your child has attended the Enrolment Mass.

If you would like your child to travel to/from the course on his/her own for the duration of the programme, please select the appropriate option below and give us your consent to release your child at the end of each session. Christchurch Eltham has a duty of care for your child under the relevant safeguarding legislation and cannot release your child unless consent has been given.*

Your child’s safety and well-being is of paramount importance to the Parish. At some events/activities we may wish to take photographs, video recordings or live-stream an activity where your child is present. Therefore, we request that all parents/carers complete and sign this consent form to give consent for the parish or their representatives to take photographs, video recordings or live stream an event/activity where your child is in attendance. If you do not wish for your child to have their photograph taken, be recorded or live-streamed, the parish will make every attempt to fully include your child, however, there may be some occasions where this is not possible. It will be explained to you if your child cannot participate.
I/We will make every effort to participate in the Confirmation preparation programme, by encouraging my/our child to attend all sessions and by supporting my/our child in becoming fully ready to be Confirmed at the end of the course. *
