Statement of Consent:- I want to enroll my child/ren in dance classes at Trenna's Dance Studio, as stated above. I understand that I am responsible in paying all fees associated with these classes, and all fees must be paid by the due date (at commencement of Term). I have included a non-refundable annual enrolment fee of $50.00 (per student)/ or $90.00 (per family), to cover the cost of insurance and I understand this is for the full year of 2025.

TERMS AND CONDITIONS
A non refundable registration fee of $50 per student (or $90 per family) to cover insurance is payable upon initial enrolment and at the commencement of each year. Enrolment forms and registration fee must be received by Trenna's Dance Studio prior to the commencement of any dance classes. Fees are charged on a term basis. Once enrolled, full term fees for all classes will be invoiced at the commencement of term.

All TERM FEES are to be PAID IN FULL at the start of term or due date on your account. Overdue accounts will incur a $20.00 late fee. Please see parents and students handbook for full details of late fees. All accounts are to be paid by direct transfer. If required, we also accept cash in an envelope with your child's name and amount on it. Direct transfer is our preferred method of payment. Failure to make payments in a timely manner or in accordance with any payment plan agreement will result in termination of student enrolment.

There are NO REFUNDS OR DEDUCTIONS GIVEN FOR MISSED CLASSES or withdrawing from classes. We do offer 'make-up' classes to students which can be taken in another style of dance, and is a great way for students to learn other styles they may not be familiar with. Please advise us prior to taking a 'make-up' class so we are able to place you or your child in a class suitable to their needs. This includes students unable to attend regular classes due to illness or injury. Students who are ill or injured but not contagious are encouraged to still come to class and watch to keep up to date with their classes.

There are no classes held on public holidays, but classes do run on pupil free days. There is no discount to term fees for classes that fall on a public holiday. Students are able to attend a 'make up' class where this occurs. Unless otherwise advised, all active enrolments will continue into each new term and will be invoiced accordingly. Should you decide not to continue into a new term, written notification must be given by the end of the previous term.

Parent Release Form for Photography and Videography

I, the undersigned, give permission for Trenna's Dance Studio to use video footage and / or photographs of my child/ren/ward.This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to:

- printed materials (eg - brochures and newsletters)

- online and offline advertising and promotion

- videos and digital images such for use on Social Media.

By signing this form, I acknowledge that I am giving unrestricted permission for my child's image to be used in print, video, and digital media. | agree that these images may be used by Trenna's Dance Studio for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.

While all due care is taken in our classes, injuries can occur. In allowing my child to participate in Trenna's Dance Studio's activities, I hereby assume all the risks associated with the performing arts. I also understand that a hands on approach is sometimes necessary to correct students' technique. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release Trenna's Dance Studio and it's instructors of any and all liability which may arise as a result of my child's participation in activities at Trenna's Dance Studio.
