Please download the Business Liabilities Form above. Fill in the form, save it, then attach it to your application below. You must disclose ALL current Business Liabilities, this includes existing loans, overdraft facilities, HP/Leasing Contracts, Invoice Discounting facilities, Debentures, Business Credit Cards, Fuel Accounts and any other line of credit utilised by the business.

If you have not already prepared any Cashflow Forecasts, please populate the spreadsheets below with the data required. You will need to categorise by specific turnover - for example; Services, Products etc, average sales price, average cost price - which will then calculate a gross margin. Do not be too specific, just focus on the bigger picture.

Please confirm "unit" sales for each product service that you anticipate for each month, starting from next month. For example, if you are going to do 4 sales of Product/Service 1 in Month 1, input 4 and so on.

Please provide ALL of the following information requested. (Aged Debtor & Creditors may not be relevant to some businesses - this is not a statutory requirement).
