CPAO's Membership Agreement is embedded in this form. You will need to sign the agreement form in order to submit your payment. Also, you will find the CPAO's Code of Ethics which you can print and/or download.

If you are a current CPAO provider, associate or professional member you may pay your 2025 membership dues online by credit card. Please note that a 3.0% processing fee has been added for credit card transactions.
