Private Appointment Form

Please fill out the entirety of this form, we want to make sure that we have all the information we need to help you find your dream dress! Please note that once the form is turned in, we will be contacting you to schedule/pay for your appointment within 48 hours.
Our Standard Private Appointment Includes:
- Up to 7 guests (upgrades available below)
- After-hours/Sunday appointments available
- 2 hour appointment
- Your choice of one celebratory beverage to celebrate your "Yes" (alcoholic or non-alcoholic)
- Your choice of music at the appointment
- A senior consultant and a second consultant to help with your group
- $200 goes towards the cost of your wedding dress
Options to include catered foods, drinks and gifts will be available to choose from below.

Appointment is non-refundable with limited rescheduling due to ordering food and/or scheduling items in advance. Private appointments are special and happen outside of scheduled store hours. Appointments must be booked 1 week in advance. The portion of the money that goes towards the dress is only valid on the same day of the private appointment and is forfeited if the bride leaves without purchasing a dress.

Appointment is non-refundable with a $100 rescheduling fee due to ordering food and/or scheduling items in advance and subject to availability. The portion of the money that goes towards the dress is only valid on the same day of the private appointment and is forfeited if the bride leaves without purchasing a dress. No outside food/drinks are allowed to any appointment type.
Once the form has been submitted, please expect a call/text within 48 business hours to make the payment for the private appointment and to confirm scheduling.

After payment is confirmed, your bridal stylist will be reaching out to you closer to your private appointment date to go over dress details.
