Waiver and Release of Liability
I, the undersigned parent or legal guardian, give permission for my child named above to participate in activities and events sponsored by CMC Church.
I understand that participation in church activities may involve physical activity, group interaction, or travel that could carry some risk of injury, illness, or accident. I knowingly and voluntarily assume all risks, both known and unknown, associated with my child’s participation.
I hereby release, waive, and discharge CMC Church, its pastors, ministry leaders, officers, employees, volunteers, and representatives from any liability, claims, demands, or causes of action arising out of or related to my child’s participation in church activities, including but not limited to personal injury, illness, property damage, or other loss, to the fullest extent permitted by law.
I agree that CMC Church is not responsible for injuries, accidents, or damages that may occur during or as a result of participation in church-sponsored activities or events.
Medical Consent
In the event of an emergency, I authorize CMC Church to obtain medical treatment for my child if I cannot be reached. I understand that I am financially responsible for any medical expenses incurred.
Photo & Media Release (Optional)
I grant permission for CMC Church to photograph or record my child during church activities and to use such images or recordings for church-related purposes, including but not limited to church publications, websites, and social media.
Acknowledgment
I have read and understand this waiver and release. By signing below, I acknowledge that I am the parent or legal guardian of the child listed above and agree to the terms stated herein.
