You voluntarily choose to undergo a spray tan service with Jaclyn Nicole Aesthetics to achieve a sunless tan on your skin. For best results, please read and indicate that you understand and agree with the following information:
- By using Jaclyn Nicole Aesthetics’ services, you release Jaclyn Nicole Aesthetics, its associates and establishment from all liability related to these services and is not liable for any injury to person or property or the loss or theft of any personal property.
- You understand the procedures, potential risks, and expected outcomes. Risks include allergic reactions, skin sensitivity, uneven tan, and temporary fabric staining. Results may vary and that no guarantees of specific results are offered or implied.
- An airbrush tan, like any self-tanning product, will not protect your skin from burning in the natural sun or in a UV tanning bed. Please take the proper precautions when exposed to UV light.
- All the components of the airbrush tanning solution have been FDA approved and safely used in cosmetics and nutritional products for decades and proven to be safe for the skin. If you have had adverse effects utilizing self-tanning products or moisturizers in the past, you may experience similar effects with the airbrush solution.
- Ingredients are generally safe but can cause allergies. Inform your technician of any allergies, medical history, or prior adverse reactions to DHA products if needed.
- If you have any history of asthma or other respiratory or medical conditions that could be aggravated by the use of the airbrush solution, please consult a physician before proceeding.
- Understand that changes in medications, pregnancy, and "that time of the month" can cause your tan to not develop as dark or last as long. Understand that if not taken care of properly, or, if you submerge in water, you will reduce the effects of the spray tan and your tan may fade much quicker.
- Jaclyn Nicole Aesthetics may use before and/or after pictures/videos on social media for promotional marketing if agreed upon by the client.
I understand and agree to comply with all instructions. I have been properly instructed prior to my airbrush session. I am using these services at my own risk. I hereby authorize Jaclyn Nicole Aesthetics to perform such tanning procedures as may be deemed necessary or advisable, and have provided them with the information required.
