Bird Owner Surrender Form

We understand that finding a new home for you pet can be a challenging and emotional decision. Every adoption application is carefully evaluated to ensure that your pet is a good match with the applicant. A rehoming agreement will be a part of the adoption process, ensuring that your pet's new family is dedicated to providing the love and care they need. We have over 35 years of pet care experience and matching people with pets. There are many reasons that people need to rehome a bird. Some outlive their current owners or they simply lose the ability to care for them effectively. What ever the reason we are here to help.

By signing below, I acknowledge that surrendered birds may be transferred to partnering rescue organizations or to approved traveling exotic animal education programs/petting zoos when appropriate. I grant Diane’s Pet Rescue full discretion to determine the most suitable placement for the animal based on its temperament, species-specific needs, and available resources. If this is an urgent placement, complete this surrender form and email diane@ddpets.com 

Diane’s Pet Rescue has generously agreed to assist you in placing your pet in a new home. The terms of this agreement are as follows: Once the pet is surrendered to our rescue, you're giving up all rights to this pet. If for any reason Diane’s decides to no longer assist you in placing your pet, you will take the animal back into your possession within 24 hours of being notified.
