57th Annual Art & Design Juried Student Exhibition Entry Form
ARTIST INFORMATION- Do not use ALL CAPS. Capitalize title appropriately and use correct spelling. This information is directly imported for exhibition labels.

STUDENTS MAY SUBMIT UP TO A TOTAL OF FOUR ARTWORKS FOR CONSIDERATION. Student will have to fill out the form for each work submitted.

Once you hit the submit button you will receive a thank you notification. Please print this out and tape it to the back of your artwork before dropping off the artwork to your faculty. Thank you! (If you were unable to print the form out, please go to your email and print the confirmation email to use instead.)
