By registering my athlete, I am authorizing participation in all and any activities prepared by Game Time Playz (Alec Martinez), D'Vontay Friga (Athlete), Gym Location, and sponsors during the basketball camp. I also hereby approve my athlete to be Photographed, Videotapped, and posted on social media for camp content or any other social media content. I also hereby understand that anyone named above, and anyone else participating in the camp, are not responsible for any injuries, lost, or stolen items during this event. In case of injruy to athlete, I hereby waive all claims against said organizations mentioned above. Inlcuding all coaches, partners, sponsors, or participants. Some of these include but are not limited to, the risk of injuries, paralysis , or death.

By acknowlding and siging below, I am delivering an electronic signature that will have the same effect as an original maual paper signature. The electronic signure will be equally as binding as an orginal manual paper signature.
