Parent Permission and Release

I, as parent/guardian of the above BCHS student, grant permission for this child to participate in the Tuition Work Program,

I hereby release and save harmless Bishop Chatard High School and any and all of its employees from any and all liability for any and all harm arising to as a result of traveling or participating in any Tuition Work Program event.

I understand that school personnel may not be present at these events or during travel to the events.

If they are present, I understand that in the event of an emergency, school personnel will make every effort to contact me. I authorize those authorities, if present, to seek emergency medical treatment in a life-threatening or serious situation.

I further understand that all school rules and regulations apply to the conduct of any school-sponsored activity away from the premises.
