    Evelyn A. Scott Foundation (EAS) is a 501(c)(3) tax-exempt nonprofit organization dedicated to providing essential resources in four key areas: Food & Nutrition, Education, Health, and Personal Essentials.

    Our mission is “Paving The Way For The Next Generation” by creating opportunities, empowering youth and families, and strengthening our community through meaningful programs and outreach.

    Thank you for your interest in serving with Evelyn A. Scott Foundation. This form is for both current volunteers updating their information and individuals who are interested in joining our volunteer team for the first time.

    Please use this form to provide or update your contact information, availability, interests, skills, and areas where you would like to serve. The information collected will remain confidential and will help us identify meaningful volunteer opportunities that align with your interests and the needs of our programs.

    All volunteer applications are subject to review and approval. Completion of this form does not guarantee volunteer placement. Volunteers may be required to complete additional steps, including background screening and required agreements, based on their volunteer role.

  • Volunteer Background Check Information

  • At Evelyn A. Scott Foundation (EAS), the safety and well-being of the youth, families, volunteers, and community members we serve is our highest priority. Certain volunteer roles may require a background check, especially positions involving direct interaction with children, youth programs, handling sensitive information, or representing the Foundation in the community. By submitting this form, you acknowledge and understand that: A background check may be required before participating in certain volunteer activities. Background checks are conducted to help ensure a safe and secure environment for all participants. Information obtained through the background screening process will be handled confidentially and used only for volunteer eligibility and safety purposes. Completing a background check does not guarantee volunteer placement. Volunteers must provide accurate and truthful information during the screening process. EAS reserves the right to determine volunteer eligibility based on the results of the screening process and the requirements of the volunteer role.

    As a volunteer with Evelyn A. Scott Foundation, I agree to help create a safe, respectful, and supportive environment for children, youth, families, and all community members.I will:Use appropriate language and behavior at all times.Never engage in bullying, harassment, discrimination, intimidation, or abuse of any kind.Maintain professional boundaries and avoid being alone with a child in situations that could be misinterpreted or unsafe.Follow all Foundation policies, safety procedures, and instructions from staff or designated leaders.Immediately report any concerns involving safety, misconduct, suspected abuse, or inappropriate behavior.Respect privacy and keep confidential any sensitive information shared with me through my volunteer role.Refrain from any physical contact that is unsafe, unnecessary, or unwanted.Never use alcohol, drugs, weapons, or any other prohibited items while serving.I understand that violating this code of conduct may result in removal from volunteer service and/or further action by the Foundation.
