Your Rights as a Client
1. Unless there is an emergency, all meetings between us will be private and confidential except for the five conditions below:
a. Any issues of abuse,
b. Expressed threats of violence towards someone specifically,
c. Planning for/or suicide attempts,
d. Sometimes I must share information to help improve your care with other people that provide services to you,
f. Requests from legal and administrative institutions.
2. With your written agreement, I may legally speak to another healthcare providers or your family members in emergencies. You have the right to tell me who I can and can't share information with and you can change your mind at any time and refuse permission.
3. I am allowed to keep brief notes of the meetings which I will keep private. You have the right to ask me at any time for a copy of my notes.
4. You have the right to ask me any questions on what and why we talk about certain things in our meetings.
5. You have the right to not participate in any of the creative projects we do in our meetings.
6. You have the right to stop meeting with me any time you wish. You also have the right to return to meeting with me any time you want.
7. I have the right to stop meeting with you I feel there are problems that we can not work through together in a positive and constructive way.
