In consideration of my engagement as a participant in the photography session on Sunday, December 22, 2024, 2-4pm, upon the terms hereinafter stated, I hereby grant to Taylor Made Voices LLC (Taylor Made Voices), its legal representatives, partners, affiliates, and assigns those for whom Taylor Made Voices is acting, and those acting with its authority and permission.

I agree and consent that Taylor Made Voices may use all or part of any photos, videotaped materials or statements provided by me for media, promotional, and/or fundraising purposes. Taylor Made Voices may have the right, title, and interest in any and all results and proceeds from said use or appearance. I understand that I, nor my child (if applicable) am not being compensated for this project.

The rights granted to Taylor Made Voices are irrevocable, continuous and include the use of this material in any collateral materials, including but not limited to, brochures, newsletters, annual reports, etc., any broadcast or print news media in any format.

a) I also consent to the use of any published matter in conjunction therewith.

c) I hereby waive any right that I may have to inspect or approve the finished product or products or the advertising copy or printed matter that may be used in connection therewith or the use to which it may be applied.

d) I hereby release, discharge, and agree to save harmless Taylor Made Voices LLC, its legal representatives or assigns, and all persons acting under its permission or authority or those for whom she is acting, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasion of privacy.

e) I hereby warrant that I am of full age and have every right to contract in my own name in the above regard. I state further that I have read the above authorization, agreement, and understand the meaning of this release.
