Introduction
This form must be filled in by all adult participants in an AOF Activity prior to the activity taking place.

It provides us with the information to:
- Effectively communicate with you.
- Invoice correctly.
- Prepare a safe, appropriate and engaging activity for all participants.
In addition to the above it is a requirement of our insurance and therefore we reserve the right to share it with them if requested but will inform you if such a request is made.

Accessibility Information
A core part of AOF's philosophy is trying to make our activites as accessible as possible (for further information please see our accessibility policy) and as such we need to collect information that will help us in that aim and cater to our client's needs as individuals. To be clear we use this information, including medical, not to try and find ways to exclude people from activites but rather to find ways for them to participate in a way that is safe and appropriate for them.

As is to be expected this data will be treated as strictly confidential and, where applicable, as special category data under UK GDPR (for further information please see our privacy policy). With this in mind please be as specific as possible and are comfortable with to enable us to fulfil the aims and objectives laid out above. If you have any questions about how we process this information please don't hesitate to ask and if any of it changes by the day of the event don't hesitate to inform your instructor/coach/guide.

I understand that mountain walking, climbing and bouldering are activities that have inherent danger and while all steps will be taken to minimise risk, these activities remain potentially dangerous. As such, I agree to adhere to any and all safety instruction.

I confirm that all equipment provided by myself is in good condition and is safe for the activity. If deemed unsafe, I agree to use alternative equipment or potentially, agree to end the session early.
