Whether you choose to apply online or download the form and fill it out by hand, you must still come in-person to pay the $30 or $50 fee. Pay fee in-person at 310 West Valley Avenue, Monday-Friday, 8:00am - 4:00pm. Payment can be by cash, check, or credit card (VISA or Mastercard only).
