TeamWork Program Application

The TeamWork Program provides direct support to youth sports teams that are working hard to give kids a safe, positive, and inclusive place to grow. Whether your team needs equipment, uniforms, coach development, travel assistance, or help covering league fees, this program is designed to remove financial barriers so athletes can focus on playing, learning, and belonging.

We prioritize teams serving military families and underserved communities, but all youth, school‑based, and community teams are welcome to apply. Please complete the form below with as much detail as possible so we can understand your team’s needs and the impact this support will make.

Through this program, CR8 Sportswear will provide gear and/or funding to teams or sports organizations serving young people in NEPA and Surrounding Regions, in the form of grants of up to $500.

Awards are issued on a rolling basis and are dependent on available funding at the time of review. Submitting an application does not guarantee support, but every request is carefully evaluated based on need, impact, and alignment with our mission. Teams are encouraged to apply early, as funds may be limited and distributed throughout the year.
