I, the undersigned, hereby grant to Michael Kinsey the absolute and irrevocable right and unrestricted permission to use and publish photographs of me taken by or on behalf of Michael Kinsey.

I understand and agree that these photographs may be used for any lawful purpose, including but not limited to:

Advertising
Marketing
Promotional materials
Social media
Publications
Exhibitions

I waive any right to inspect or approve the finished product or any other use of the photographs. I further waive any right to royalties or other compensation for the use of the photographs.

I release and hold harmless Michael Kinsey and their representatives from any and all claims, demands, and causes of action arising out of or in connection with the use of the photographs.

I declare true my age of consent, and in case I am under the age of such consenting age, I have obtained the required consent from my parents/guardians as evidenced by the signatures below:
