Please type the Titling Program, Title Category, Title Track (if applicable), and Levels for each title that you are submitting for. You may submit for titles in separate title programs or categories through one entry form. However, a separate entry form must be submitted for each dog.
TITLING PROGRAMS - Adventure Puppy, Classic, Specialty, Specialty Extreme, etc.; TITLING CATEGORIES - Jolly Pup, Wagging Walker, Sweet Pup, etc.; TITLE TRACKS - Trails, Distance/Hours, Outings, Locations, etc.; LEVELS - 1, 2, 3, 4, Huron, Erie, etc.

I understand that outdoor activities including, but without limitation, hiking, walking, boating, or camping, is not without risk to myself, members of my family, my guests, or dog(s). By submitting this entry, I hereby waive and release “Outdoor Dog”, it’s company “The Bouncing Beagle Co”, and owner Stephanie Peebles from any and all liability of nature for injury or damage which I or my dog(s) may suffer. I expressly assume the risk of such damage or injury while participating in “Outdoor Dog” under the agent of Stephanie Peebles. In consideration of and as inducement to the acceptance of my entry to “Outdoor Dog”, I hereby agree to indemnify and hold harmless “Outdoor Dog”, its company “The Bouncing Beagle Co”, Stephanie Peebles, and all employees and agents from any and all claims by me, or function of “Outdoor Dog”, as a result of any action by any person(s), including myself, or any dog(s), including my own.
