I have read the Exclusive Hiring Hall Rules referred to in the Labor Agreement between the AGC. and Teamsters Local Union No. 690, and I hereby register as provided, and certify that I am qualified to preform the duties required in the classifications checked below, and I am not registered on any other construction list within the geograpgic area of this Agreement.

(List proof of at least two years of actual experience in the building, heavy and highway construction industry)
