If you do not have ALL of the documentation uploaded your appliation will be DENIED. Make sure you have the following scans or pictures:
- Parent/Guardians Drivers License/ ID
- Proof of residence if different from ID (Business mail addressed to parent/guardian or lease)
- Proof of Income (Check Stubs, 2024 Tax Return, Employer letter stating salary, documents of child support. Award letter for TANIF, Food Stamps (SNAP, Unemployment or Workers Compensation.)
- EACH Childs Birth Certificate (or other official document that verifys date of birth)
- EACH Childs proof of School Enrollment (Report Card or dated letter from school verifying enrollment)
Again without these items uploaded your application MUST be DENIED.
